Medical Records
The Medical Record Specialist processes all requests for release or transfer of medical records. Questions concerning the release or transfer of medical records should be directed to that extension between the hours of 8:00 a.m. and 4:00 p.m. Monday through Friday.
In keeping with our Privacy Practices and Privacy Law:
- We require a signed authorization to release information before medical records are copied and forwarded to the designated party. The patient or his/her legal representative must sign this form.
- Requests are accepted by fax or mail, as long as, the appropriate signed authorization to release information is included. Please see the top of this page for fax and address information.
- Patient medical records are the property and possession of Gainesville Family Practice, P.C. However, as the patient, the information contained in the record is yours and you have a right to access that information.
- We will not release medical information without a patient’s signed authorization.
- There will be a fee for the copying of records. The fee is based on the number of pages and includes and handling fee and postage if necessary. Please contact our medical records department for further information regarding these fees.
Records transfer from other physicians:
- It is the responsibility of a patient to arrange for the transfer of medical records from their prior physician to our practice. We are happy to provide you with an Authorization for Release of Medical Information form to sign and send to your previous physician/healthcare provider.
